The software package provides a
structured environment in which to collect and assemble facts
from the investigation, such as what and who was involved. The
primary tool for this is the graphical Tripod Beta tree, which
allows a visual representation of the incident to be created and
manipulated on the screen. The tree consists of nodes of
different types, and connectors between nodes which show how
nodes are related. Constructing trees is simplified by the use of
a wizard.
Within a Tripod Beta tree, the key items for the investigators
are shortcomings in the Management System—factors in the
prescribed way of working that make incidents inevitable when
other conditions are right. Identifying these, and suggesting
remedial action, is the ultimate goal of an investigation.
While a Tripod Beta tree is being
created or manipulated, the software continuously checks the tree
against a set of rules to ensure that it is logical. The rules
used are derived from the Agents of change and Effects Management
Process (HEMP) and the Tripod theory of accident
causation.
After the incident has been investigated and all the information
has been organized, the software will, on request, generate a
draft report containing all salient facts of the incident. The
report can be made in many formats, including MS-Word, so
additional information can be added outside the Tripod β V4
environment. In addition to presenting the information you enter
in a narrative format, the report includes an analysis of the
failures identified in the Management System.
Extra material, such as reports of interviews, pictures, and
presentations, can be stored together with the investigation to
make a logical whole.
